By Car:

From Verrazzano Bridge

1. Take Verrazano Bridge to 92nd Street exit. Turn left at corner.
2. After 3 blocks, turn right onto 4th Avenue, and proceed to Atlantic Avenue
(approximately 5 miles).
3. Turn right onto Atlantic Avenue and continue past Utica Avenue. (Notice cutout under overhead railroad).
4. Pass under railroad and take first left onto Rochester Avenue.
5. Continue one block, then turn right onto Herkimer Street.

From Five Towns Area

1. Follow Rockaway Parkway Boulevard straight until sighting sign for Belt Parkway West, then follow Belt Parkway until Exit 17W.
2. Take conduit to Atlantic Avenue and continue all the way to Suydam Street, one block past Buffalo.
3. Turn right onto Suydam Street. Continue one block, then turn right onto Herkimer Street.

From The Bronx

1. Take Whitestone Bridge to VanWyck Expressway to Jackie Robinson (Interboro) Parkway.
2. Continue to Pennsylvania Avenue (last exit) and turn right onto Atlantic Avenue.
3. Turn right onto Suydam Street (one block past Buffalo), then turn right onto Herkimer Street.
4. Take Triboro Bridge to Grand Central Parkway. Take Grand Central to Jackie Robinson (Interboro) Parkway.
5. Continue to Pennsylvania Avenue (last exit). Turn right onto Atlantic Avenue.
6. Turn right onto Suydam Street (one block past Buffalo), then turn right onto
Herkimer Street.

From Jamaica, Queens

1. Take Verrazano Bridge to 92nd Street exit. Turn left at corner.
2. After 3 blocks, turn right onto 4th Avenue, and proceed to Atlantic Avenue

1. Take A or C train to Utica Avenue, and follow exit to Fulton Street.
2. Walk 1.5 blocks on Fulton Street, then turn right onto Rochester.
3. Turn left onto Herkimer Street.

 

FACILITY VISITOR POLICY

We are happy to announce that visitation is open for all residents. 

Please keep in mind that all visits will have to follow CDC guidelines for infection control, which can include, but are not limited to the wearing of PPE and social distancing. Visitors that are exhibiting signs or symptoms of Covid-19 or that have tested positive for Covid-19 should not come to the facility regardless of vaccination status.

Visitors should be aware that there are risk factors associated with visiting residents that are on quarantine or transmissions based precautions.

Should any visitor develop signs or symptoms or test positive for Covid-19, they must contact the facility immediately.

While you can visit at anytime, we recommend visits between 10AM and 8PM.
